Use this form when your team needs protocol-grade responsibility object creation, usage terms, retention, verification, audit export, or downstream TAL-compatible handoff support.

This form does not automatically issue an L2 API key, activate billing, create a subscription, or approve protocol access. CLARIXO reviews the request before creating a protocol access profile.
